Documenting Residency Activities

Main Section

  1.  Select PxDx from the top menu ribbon
  2.  Select Add New
  3.  Enter the appropriate information in the Main section fields (* = Required Field)

Patient Assessment Section

There are three Patient Assessment Groups (A) under the Patient Assessments tab:

  • Drug Therapy Problems
  • Medication Classes
  • Pharmacotherapy Categories

Each Patient Assessment Group contains Patient Assessment Subgroups (B).  The specific Patient Assessment items from each Subgroup are populated once a Subgroup is selected. 

Patient Assessment Tab:  Selecting Drug Therapy Problems

  1. Scroll down to the Patient Assessment Section
  2. Select the Drug Therapy Problem subgroup from the Group dropdown
  3. Select the correct Drug Therapy Problem from the Patient Assessment box
  4. Select your Role (this defaults to Addressed for all of your selections under the Patient Assessments tab)
  5. Enter any Notes specific to the selected drug therapy problem (optional)
  6. Select Add Patient Assessment

More than one Drug Therapy Problem may be selected.
 

Enter text in the Filter box to do a keyword search
 

Patient Assessment Section:  Selecting Medication Classes

  1.  Select the Medication Classes subgroup from the Group dropdown
  2.  Select the correct Medication Class from the Patient Assessment box
  3.  Select your Role (this defaults to Addressed for all of your selections under the Patient Assessments tab)
  4.  Enter any Notes specific to the selected medication class (optional)
  5. Select Add Patient Assessment

Patient Assessment Section:  Selecting Pharmacotherapy Categories

  1. Select the Pharmacotherapy Categories subgroup from the Group dropdown
  2. Select the correct Pharmotherapy Category from the Patient Assessment box
  3. Select your Role (this defaults to Addressed for all of your selections under the Patient Assessments tab)
  4. Enter any Notes specific to the selected pharmacotherapy category
  5. Select Add Patient Assessment

Interventions Section

  1. Scroll down to the Interventions section 
  2. Select the appropriate Intervention subgroup from the Group dropdown (only select interventions from the PRACTICE RELATED intervention group)
  3. Select the appropriate Intervention (activity) from the Intervention box
  4. Select your Role in the activity from the dropdown menu
  5. Select the Intervention Duration (optional)
  6. Enter any Notes specific to the selected intervention (optional)
  7. Select Add Intervention
  8. Select Save Record (if you do not save all of your information will be lost)

Multiple interventions may be selected
 


Templates

At the bottom of the page you have the option to save your entry as a template by selecting Save as Template. Once selected, you will be prompted to name the template. To access your templates, scroll to the bottom of the page and select your template from the Template dropdown.

All items that you have selected are saved in a template - including the Date. Make sure to change your date whenever you use the template.
