The ideal candidate will have completed or is completing residency program by June 2016. Experience during rotations working with underserved patient populations with a large emphasis on diabetes and other chronic illnesses.

The Siouxland Community Health Center (SCHC) has been a part of Sioux City for over 20 years and maintains certification as a level three Patient Centered Medical Home (PCMH). We are a well-established community leader within the healthcare field providing a variety of services for our patients including primary/preventative medical and dental care, behavioral counseling, HIV treatment, urgent care, a 340B pharmacy, an in-house lab, case management, onsite interpreters, and clinical pharmacists. We take pride in providing comprehensive, affordable healthcare services to our community and are known statewide for the level of quality we maintain throughout the organization. Hours open M-F 8:00 - 5:00. Generally sees 12 patients per day.

The position collaborates closely with primary care providers and case managers. This position also has teaching responsibility through UNMC which includes guest lectures and providing guidance during clinical rotations. Full on-site pharmacy.

Successful candidate must have solid experience and advanced knowledge of:

Doctor of Pharmacy degree from an accredited school of pharmacy
Complete residency and/or fellowship training in ambulatory care, managed care, primary care, community pharmacy or pharmacoeconomics/outcomes through an accredited pharmacy residency
Specialty board certified pharmacist preferred
Trained in clinical pharmacy, clinical pharmacokinetics and clinical pharmacology
Holds and maintains a valid pharmacist license in the State of Iowa

Successful candidate must be able to perform primary functions of position:

Collaboratively work with medical staff to manage patients with chronic diseases.
Provide patient and health care professional education and medication information.
Recommend medications, including initiation, continuation, discontinuation, and altering therapy
Through University of Nebraska Medical Center (UNMC), serve as Clinical Assistant Professor, provide lectures on site at UNMC, precept fourth year pharmacy students on site at SCHC.
Serve as clinical advisor of drug and drug-related programs in the clinic in conjunction with the attending physician.
Assist SCHC staff to ensure patients are able to obtain medications in the most cost effective manner possible to improve medication adherence.
