Ann Schwemm is a clinical product and content specialist manager at Flatiron Health, Inc., a cancer technology company connecting community oncologist, academics, hospitals and life science researchers and regulators on a shared technology platform.
Prior to joining Flatiron, Schwemm practiced as a clinical pharmacist at the University of Minnesota Masonic Cancer Center in Minneapolis as well as the University of Washington (UW) Seattle Cancer Center in Seattle. She served as the initial pharmacist for the UW Medicine ACO and Population Health Program. She has also worked as a consultant pharmacist for MyMeds, Inc providing clinical insight and data analysis to demonstrate improved medication adherence in users of the MyMeds program.
Schwemm is a member of the Minnesota Pharmacy Association, the Hematology and Oncology Pharmacy Association, American Society of Health-System Pharmacists, and the National Community Oncology Dispensing Association, Inc. She graduated from the University of Northern Iowa in 2005 and the University of Iowa College of Pharmacy with her Doctor of Pharmacy degree and Master of Public Health degree in 2009. After completing her PharmD/MPH, she pursued a post-graduate year 1 pharmacy practice residency at the University of Michigan Health System as well as post-graduate year 2 in oncology. She has also completed certificate programs in Medication Quality through the IHI and Health Economics and Outcomes Research through the University of Washington.
