1. Select the Home tab
  2. Select Other Tasks
  3. Select Manage Personal Records

 

  1. Enter the date of the certification or immunization under Event Date
  2. Check the Expiration Date (this defaults to one year from the Event Date)
  3. Upload an electronic copy of the certificate or immunization record using the Add attachment field
  4. Select Add

*Select the edit icon to edit a record that has not yet expired.
