1. Select the Professional Development Activities program
  2. Select Continue Login

  1. Select Case Logs from the top menu ribbon
  2. Select Log New Case Entry

  1. Date of Experience: Enter the date of the activity
  2. Number of Patients Encountered: When applicable, enter the number of patients
  3. This encounter involved other health care providers: When applicable, select other health care providers involved in the experience
  4. Organization: Select the appropriate Organization for the activity from the dropdown.
  5. Time Involved: Select the amount of time at the activity (this should match what is on the Verification/Meeting Attendance form)
  6. Group: Select the type of activity from the dropdown (PROFESSIONAL SERVICE, PROFESSIONAL LEADERSHIP, COMMUNITY ENGAGEMENT)
  7. Activity: Select the activity from the select box
  8. Select Add Activity
  9. Select Save Record
