Criminal Background Check

DUE: Upon acceptance of admission offer

As part of your conditional offer of admission, you must provide consent for the University of Iowa College of Pharmacy to view your criminal background screening report. The University of Iowa does NOT require the drug screening. Please follow the instructions below:

  • You will receive an email from Certiphi after you accept your offer of admission. The email will contain the URL and a code to enter in the Application Station Code field where prompted.  NOTE: If you cannot find the email in your inbox, it may be in your spam folder.

NOTE: Failure to provide consent may result in failing to meet the University of Iowa College of Pharmacy's admissions requirements.  Issues arising from the Background Investigation report may need to be resolved in order for your admission to be finalized, and your cooperation with the College of Pharmacy in this regard will be essential. The College of Pharmacy reserves the right to rescind its offer of admission on the basis of the background report (provided that you have had the opportunity to respond).

Student Health Requirements for Health Science Students

Start this process NOW (upon admission to the program)!  Some of these Student Health requirements take months to complete.

If you have any questions regarding these health requirements, visit the University of Iowa Student Health website, contact via email immunizations@healthcare.uiowa.edu or by phone at 319-335-8370 (tell them you are a "Fall 2022 health science student in Pharmacy").

Health Insurance Requirement

START: Now

DUE: September 9

All students at The University of Iowa, including Pharmacy students, are required to have health insurance.

  • If Pharmacy students want to enroll in a University student health insurance plan, there are two plans from which health science students may choose (SHIP and UIGRADCare).
  • Otherwise, students must submit proof of health insurance coverage beginning August 1 and no later than September 9 to avoid being automatically billed for the Student Health Insurance Plan (SHIP).

ICON (Iowa Courses Online) Course Invitation

OPEN: May

You will receive an email invitation to the ICON/Canvas course "Compliances - Class of 2026" at your University of Iowa email address. Please accept the course. Instructions to complete various PharmD requirements will be posted on the "Modules" page and are due by the dates listed.

  • START HERE: "Accept" course invitation in message sent to your @uiowa.edu email address and follow instructions to complete all requirements.
  • NEXT: ICON/Canvas >> Canvas Home (yellow ribbon) >> Compliances - Class of 2026 (select "modules" for instructions and links to training).
  • NOTE: Completion start and due dates vary, so check back often to ensure you complete assignments well before deadlines.

Compliance & Qualification (C&Q) Training

OPEN: May 15 (multiple courses - approximately 14 hours to complete)

DUE: July 1

College of Pharmacy students are required to complete IPPE/APPE (Introductory/Advanced Pharmacy Practice Experiences) Patient Care Practicum Compliance Training with self-registration through the Compliance & Qualifications (CQ) system (see below to get started).

There are multiple training modules that take approximately 14 hours total to complete, so get started as soon as possible.

Basic Life Support (BLS) for Health Care Providers

COMPLETION PERIOD: May 15 - July 1

Iowa law requires documentation of current certification in basic cardiac life support through a training program designated for health care providers that includes hands-on training.

The College of Pharmacy accepts the following BLS training certification options:

  • American Heart Association (AHA) Basic Life support (BLS) -- full classroom course
  • American Heart Association (AHA) HeartCode BLS -- blended learning course (online with a hands-on skills session)
  • American Red Cross (ARC) Basic Life Support for Health Care Providers – in-person or blended learning

TRAINING OPTION 1 (preferred):

  • START HERE:  ICON/Canvas >> Compliances - Class of 2026 (select "modules" for instructions and links to training).
  • COMPLETE:  Take quiz and upload PDF certificate on ICON/Canvas >> Compliances - Class of 2026.

 

TRAINING OPTION 2 (advance registration required):

Students may register for the AHA HeartCode BLS (on-site hands-on skills portion only) offered at the College of Pharmacy during Orientation/Engagement Week (usually offered during the evening). Space is limited.

  • TO UTILIZE TRAINING OPTION 2 STUDENTS MUST: 
    • EMAIL: Email both Dr. Jeanine Abrons (jeanine-abrons@uiowa.eduand the Office of Professional Education (pharmacy-ope@uiowa.edu) no later than July 1 to secure a space in the on-site hands-on skills portion of the course.
    • REGISTER: Upon email confirmation from Dr. Abrons or the Office of Professional Education (OPE), register for the online web-based portion of the course at American Heart Association HeartCode BLS (https://shopcpr.heart.org/heartcode-bls).  NOTE: Payment for the online training portion of the HeartCode BLS is due upon online registration with the AHA. Payment for the hands-on session will be collected separately at the beginning of training at the College of Pharmacy.
    • ONLINE TRAINING: Complete online training and upload the online web-based certificate to the ICON quiz completing questions #1 and #3 between July 1 and August 1.  You will receive full quiz points after the hands-on skills session. NOTE: Hands-on skills training must be completed within 60 days of the online training. Do not take the online training prior to July 1.
    • ON-SITE SKILLS TRAINING: Be prepared to make $40.00 cash or check payment for the on-site hands-on skills portion of the training offered at the College of Pharmacy training site. NOTE: This amount is subject to change and is in addition to the amount previously paid to the AHA for the online portion of the training.
    • COMPLETE: Upload final hands-on skills session certificate (emailed after the completion) to ICON/Canvas >> Compliances - Class of 2026 (ICON/Canvas access instructions noted above).

Professional Engagement Course and Orientation

CLASS BEGINS: Week prior to UI Opening of Classes (initial week plus longitudinal course in P1 Fall)

Engagement: Professional Skills & Values (PHAR:8131)

Class begins on Monday of the week prior to the UI Opening of Classes for incoming P1 students. The initial week of the course includes both foundational topics that students will encounter throughout their pharmacy education as well as orientation components to familiarize students with the College of Pharmacy and other resources. Students may have some required Engagement course assignments due prior to arriving for Engagement Week (week prior to UI opening of classes).

  • COURSE INVITATION: You will receive a course invitation via your @uiowa.edu email account in early summer. Please "accept" the course invitation.
  • ASSIGNMENTS: Assignments are part of the course grade and should be completed by the due dates as required by Dr. Jeanine Abrons. NOTE: Some assignments may be due prior to Engagement Week (week prior to UI Opening of Classes).

 

Orientation (components within the Initial Week of the Engagement Course)

  • BRING WITH YOU:
    • Social Security Number (SSN). NOTE: If you do not have a SSN, please work with Patricia McCormick (patricia-mccormick@uiowa.edu or +1 319 335 8795) before or upon arrival to Orientation.
    • If you worked in a pharmacy and/or some other state licensed position, bring:
      • License number for any professional license or registration, e.g. CPhT license number (certified pharmacy technician license), pharmacy support person, nurse, nurse aid, etc.
      • Organization name(s) and address(es) where you worked.
      • Organization license number (Every pharmacy has a number.  If you worked at more than one, please bring all of them.)
      • Exact date(s) of hire and employment.

2021 - 2022 Seasonal Influenza Vaccination

COMPLETION PERIOD: September 1 - November 30

Influenza vaccination information is required for several reasons:

  • Many health care facilities require an influenza vaccination unless there is a medical contraindication.
  • The UI College of Pharmacy (UICOP) is required to report the influenza vaccinations of its students and student volunteers to UIHC. Similarly, UIHC is required to include the UICOP information in its report about healthcare personnel influenza vaccination rates to the Centers for Medicare and Medicaid Services (CMS) and The Joint Commission.
