Students are responsible for completing and maintaining compliance with all PharmD Program Requirements as outlined in the PharmD Student Handbook.


Requirement Compliance & Tracking

The College of Pharmacy utilizes CORE ELMS to provide information and track completion of PharmD Program Requirements as well as any additional practice site requirements. Students should document requirement completion in the   Requirements module of CORE ELMS.

Viewing/Updating CORE ELMS Requirements:

  1. Log in to CORE ELMS using your HawkID and password.
  2. Select    Requirements in the left menu ribbon.

Certifications & Trainings

Basic Life Support (BLS) Certification for Healthcare Professionals

Students must maintain American Heart Association (AHA) BLS certification for healthcare professionals.

UI Health Care Online Trainings

Students must complete UI Health Care Online Trainings in the Compliance and Qualifications system. See the List of Required UI Health Care Trainings and Instructions below.

  • H01897: Annual Combined HIPAA / Fraud, Waste, and Abuse Training (annual)
  • H00403: New Hire Orientation: Students and Part-Time Staff (once)
  • H00446: Domestic Violence (once)
  • H00448: Patient and Staff Rights and Responsibilities (once)
  • H02037: Safety/Infection Control (Initial) (once)
  • H02038: Safety/Infection Control (Renewal) (annual starting P2 year)
  • H01898: Mandatory Reporter: Dependent Adult Abuse Training (every three years)
  • H01899: Mandatory Reporter: Child Abuse Training (every three years)

Instructions for Completing UI Health Care Training

Step 1: Enroll in Course
  1. Go to My Training in the UI Compliances and Qualifications site
  2. Enter the course number (H0XXXX) in the Course Number field
  3. Select Search Courses
  4. Select View Details next to the course name
  5. Select Enroll in Session

Step 2: Complete the Course

  1. Go to My Courses in the UI Compliances and Qualifications site
  2. Select the course name to complete the course
    To successfully complete a course, you must score at least 80% on each quiz. You may retake quizzes if necessary.
Step 3: Verify Completion
  1. Go to My Courses in the UI Compliances and Qualifications site
  2. Select the Past Courses tab
  3. Verify the most recent course completion is listed with a status of Completed
  4. Verify the Completion Date (make sure it is for the current year)

Health Information

Entrance Health Screening

Entering students must submit a completed Health Screening Form. The form can be completed by their primary care provider or by Student Health. Upload the completed form to CORE ELMS (do not send to Student Health).

Student Health Requirements

Students must meet all Student Health Requirements outlined for Health Science Students. Students must submit this documentation to Student Health as well as update CORE ELMS.

List of Student Health Requirements

  1. Hepatitis B Vaccine Series / Titer
  2. Measles, Mumps, and Rubella (MMR) Vaccine Series / Titers
  3. Tetanus, Diptheria, and Pertussis (Tdap) Vaccine (ongoing)
  4. Tuberculosis (TB) Screening (annual)
  5. Varicella Vaccine Series / Titer

Submitting Documentation to Student Health

Students may submit their records to Student Health using one of the following methods:

UI Health Care Reporting: Influenza Vaccination Status

Students report their seasonal influenza vaccination status each year.

This is not considered proof of vaccination. Students may still need to provide proof of their seasonal influenza vaccination for some practice sites.
